INDIVIDUAL COMMUNITY LIVING SUPPORT (ICLS)

ASSISTANCE WHEN YOU NEED IT, IN THE COMFORTS OF YOUR OWN HOM

Individual Community Living Support (ICLS) is a service for Elderly Waiver (EW) and Alternative Care (AC) participants. ICLS allows clients to take advantage of services and supports under one provider, while staying in the comfort of their own home.

OUR ICLS PROGRAM OFFERS SERVICES AND SUPPORTS SUCH AS:

Activities of Daily Living (ADLS)

ADLs are basic self care tasks that an individual does on a day-to-day basis. Common ADLs that Advanced health team of caregivers can assist with include dressing, grooming, bathing, and transferring (walking).

Household Management Assistance

ICLS caregivers assist with meal planning and preparation, light housekeeping, laundry and budgeting/money management. Household management assistance may also include transportation when other supports are unavailable.

Health, Safety, and Wellness Oversight

Advanced health team of caregivers identify any changes in the client’s health, including monitoring existing health conditions, with referrals to case managers as needed. Our caregivers also assist with implementing a safe living environment, such as reducing fall risks, and assisting with medication reminders.

Community Living Engagement

Community living engagement services help clients access resources, services, and activities outside the home. These services help the client develop and maintain a support system in the community. Caregivers can provide transportation to outside resources when other supports are unavailable.

Adaptive Support

Adaptive support works to encourage self-sufficiency and reduce the need for assistance. ICLS caregivers work with the client to develop and practice individualized strategies that promote independence, such as using calendars, making lists, and teaching how to use assistive technology.

Active Cognitive Support

Active cognitive support addresses issues and challenges that are important to the client. ICLS caregivers provide in-person or remote check-ins to provide support, guidance and problem-solve concerns related to daily living.
